BASE SALARY: $211,328 to $333,673

THE ROLE:
  • The Director, Care Management provides strategic and operational leadership for all care-management functions across the acute-care ministry, including case management, utilization management, discharge planning, social work services, and psychosocial interventions that support safe, effective, and sustainable transitions of care.
  • This leader is accountable for patient outcomes, satisfaction, throughput, and care continuum utilization while driving performance that meets or exceeds regional and national benchmarks.
  • The Director develops and executes ministry-specific strategic and operational plans, manages key clinical, operational, and financial metrics, and ensures services remain patient-centered and aligned with the Triple Aim.
 
What You'll Do:
Care Management & Transition Leadership
  • Provide strategic oversight of care transitions, utilization management, case management, discharge planning, social work, and psychosocial intervention programs.
  • Lead efforts to optimize patient throughput and effective utilization of resources across the continuum of care.
  • Ensure safe, sustainable, and patient-centered discharge planning and care-transition processes.
  • Collaborate with Physician Advisors and medical staff to promote appropriate utilization practices and regulatory compliance.
Operational Excellence & Performance Improvement
  • Develop and implement annual strategic work plans that include quality, clinical, administrative, and financial objectives.
  • Monitor and manage departmental performance metrics, ensuring achievement of established goals.
  • Proactively identify and address performance trends, operational challenges, and opportunities for improvement.
  • Drive initiatives that improve quality, outcomes, patient experience, and care delivery effectiveness.
  • Support organizational goals aligned with the Triple Aim.
People Leadership & Workforce Development
  • Directly supervise and develop leaders and caregivers within areas of responsibility.
  • Recruit, mentor, coach, and retain high-performing teams.
  • Provide ongoing education, performance management, evaluation, and professional development opportunities.
  • Foster a culture of learning, collaboration, accountability, and excellence.
Physician, Executive & Community Collaboration
  • Build strong relationships with physicians, hospital leadership, and community care partners.
  • Serve as a liaison to organizations involved in care transitions and post-acute services.
  • Promote collaboration that enhances patient experience and care coordination.
  • Support stakeholder engagement through education, presentations, and performance reporting.
Financial Stewardship & Accountability
  • Develop and manage departmental budgets, staffing plans, productivity targets, and operational metrics.
  • Monitor financial performance and address budget variances proactively.
  • Ensure responsible utilization of resources while maintaining quality outcomes.
Governance, Compliance & Reporting
  • Ensure compliance with applicable licensing, accreditation, regulatory, and organizational standards.
  • Lead reporting and communication efforts regarding care management outcomes and performance.
  • Seek executive partnership and support when addressing complex patient care or operational challenges.
  • Participate in and chair/co-chair hospital and medical staff committees as required.

REQUIREMENTS:
  • Bachelor's degree in nursing AND a master's degree in a health-related/science field. 
  • At least 2 years of experience leading case management teams
  • care management / case management experience in an acute-care setting.
  • California Registered Nurse (RN) License upon hire
  • Strong knowledge of acute-care case management principles and practices.
  • Understanding of evidence-based practice and care-transition models.
  • Knowledge of the care continuum and effective transition-of-care strategies.
  • Strong problem-solving, communication, writing, and organizational skills.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Demonstrated ability to build and maintain collaborative relationships across disciplines.
  • Proficiency in Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook.
