OPEN JOB: Clinical Nurse Manager LOCATION: San Jose, California ***Relocation Assistance Available BASE SALARY: $157,023 to $207,270 FULL BENEFITS
We are seeking Clinical Nurse Managers that ideally have outpatient/clinical experience. If you’re ready to join a holistic care team that values both its participants and providers, we’d love to hear from you!
Our client’s PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities.
Under the direction of the Clinical Nurse Director (CND), the Clinical Nurse Manager organizes and directs the day-to-day operations within the clinic, oversees clinic staff, and carefully coordinates clinic workflows to ensure services are provided effectively.
The Clinical Nurse Manager is well-versed in people development, management, and RN-level nursing care.
WHY THIS JOB IS DIFFERENT:
Work a consistent Monday through Friday schedule – no weekends, no nights, and no on call, along with far less burnout compared to hospital management roles
Work with purpose and have the opportunity to observe participant outcomes over time, rather than traditional roles that treat episodes
Responsibilities:
Operate a high-performing, well-equipped, and safe clinic, as well as ensure workflow efficiency, clarity, and protocols
Hire, retain, coach, develop and supervise RN Case Managers, Clinic RNs, Medical Assistants, and other clinic team members, as well as ensure they are engaged and healthy
Analyze data with attention to trends, strengths, weaknesses, and opportunities, performing root cause analyses, evaluating care delivery processes, and making recommendations to the CND on strategies, workflows, and policies to continuously improve clinic operations
Ensure nursing and clinic staff accurately deliver and document interventions as agreed upon in participant care plans, including but not limited to promptly responding to physician orders, completing assessments, and actively participating in care planning
Evaluate the quality and effectiveness of coordination and services in conjunction with Medical Practice leadership, Providers, Case Managers, and other relevant interdisciplinary team disciplines
Serve as the audit lead for clinical and nursing elements during CMS/DHCS audit review, in collaboration with the CND and Quality Improvement Director
Develop and monitor corrective action plans to ensure ongoing compliance
